Requesting a Copy of Form 1095-C
A copy of Form 1095-C is available upon request for individuals who are enrolled in the company’s health plan and who do not automatically receive a mailed copy, including:
- Non-full-time employees enrolled in the plan
- Nonemployees enrolled in the plan (such as retirees, former employees, COBRA participants, or other covered individuals)
To request a copy of Form 1095-C, please contact us using one of the methods listed below. Requests may be submitted at any time, and a copy will be provided within a reasonable timeframe.
